A damning internal report on Cruise’s handling of an October accident in which one of its self-driving taxis struck and severely injured a pedestrian in San Francisco blames “deficient leadership at the highest levels of the company,” including members of the C-suite and legal department.

The 195-page report, issued by law firm Quinn Emanuel Urquhart & Sullivan on behalf of Cruise and parent General Motors, describes an executive and legal team that did not take command of the situation, failed to fully grasp the gravity of the crisis, and failed to ensure Cruise fully complied with its reporting requirements to the National Highway Traffic Safety Administration and other regulators.
