As of March 24, 2014, all federal contractors will be required to use a new Voluntary Self-Identification Form CC-305 [PDF] issued by the Office of Federal Contract Compliance Programs (OFCCP). The form invites applicants and employees to report their disability status.

Attorneys at Ballard Spahr have offered a detailed outline of the information requested on the form. It will notify applicants and employees that any information given cannot be used against them, and will also inform them of the employer’s obligation under federal law to provide accommodations.
