A veteran family law attorney whose license had already been suspended since 2013 was disbarred Thursday by an upstate appeals court that considered seven new professional misconduct charges against him, including his alleged failure to file a divorce judgment on behalf of his client, submitting false information in regard to his insurance agent license application and renewal forms, and failing to cooperate with an attorney grievance committee’s investigation of his actions.

Daniel J. Gallagher, who was admitted to the state bar in 1994 and whose most recent business address was in Glens Falls, in Warren County, has been disbarred by the Appellate Division, Third Department.
