The doctor for the alleged incapacitated person has completed the written interrogatories and is ready to have the documents notarized. The doctor has requested that a volunteer notary travel to her office on a week day at lunchtime to notarize the documents. Her office is located at 7133 Roosevelt Boulevard, Philadelphia, PA 19149.
