The Institute of Medicine published its long-awaited report based upon work of the Committee on Patient Safety and Health Information Technology, Board on Health Care Services. The Institute of Medicine is remembered for its blockbuster 1999 report “To Err is Human,” which pointed out the high number of preventable medical deaths in hospitals nationwide.

The revelation that the number of patients who die in U.S. hospitals from preventable medical errors each year — equivalent to two 747s crashing each month with all those aboard dying — was the beginning of the patient safety movement.
